Geriatric Care Management

With the increasing number of aging citizens, older Americans find themselves in various stages of declining health with fluctuating levels of need for assistance in planning, accessing and evaluating their personal, social, legal and medical conditions.

With training in gerontology, social work, nursing or counseling, and—most importantly—a sincere desire to assist the aged population, the elder care manager is a professional who specializes in assisting older peoplle and their families with long term care arrangements.

Care managers conduct care-planning assessment to identify problems, eligibility for assistance and the need for services. Crisis intervention, counseling and support are among the skills of the geriatric care manager (GCM).
