The job of facility review and assessment is one of diligence and thoroughness. The care manager is tasked with creating a complete evaluation of a care facility covering a broad and comprehensive list of criteria, including:
- Facility Description:
Names and accreditation of owners/administrators, licensure, size (# of beds), facility age, cost per bed, services included with costs, optional services/costs, other fees, complaint histories, admission requirements, meal services, staffing levels/credentials.
- Facility Review:
General interior and exterior appearance, maintenance schedules, interior and exterior lighting, laundry options, personal grooming service availability, social/physical activity schedules, appearance/demeanor of staff, room amenities (windows, individual thermostats, door locks, furnishings, smoking/non-smoking), evaluation of food services (multiple choices available, special diet availability, general smell/taste of food, availability of snacks, room service opportunities).
Further evaluation will be made based upon the type of facility being reviewed. As the level of skilled care services increases, additional information will be gathered to assess the capability of the stated services to be provided adequately.
